Hospice care is provided
Most people prefer to stay at home, but care can also be provided in a nursing home, assisted living facility, hospital setting or hospice in-patient unit. (Short breaks for the patient's caregiver may also be available and arranged through hospice.) Visits will be scheduled on a regular basis. More >>

The purpose of Advance Directives
is to allow you to make your wishes known when you are unable to voice them yourself. Forms are available upon request. Give a copy of the completed Advance Directives to your care team, as well as your doctor. More >>
